Treatment of Lipedema

The treatment of lipedema is an important area of focus at ReSculpt Clinic. Lipedema is a complex and often difficult to recognize condition of the adipose tissue. Because this condition is often associated with late recognition and treatment that is not optimal, lip edema can lead to frustration, restriction of movement, pain symptoms, social limitations and isolation, among other things.

Lipedema should take a multidisciplinary approach. This means that for optimal results, multiple aspects of lipedema should be addressed simultaneously. Liposuction (the "sucking out" of fat) under local anesthesia (tumescent fluid) is one of them, and also the only scientifically proven treatment that can bring about a visible and permanent reduction in fat volume. What is Lipedema?

Lipedema is a fat metabolism disease that occurs almost exclusively in women. A condition in which over time, usually starting at puberty, a painful swelling of fatty tissue develops. Lipedema occurs particularly in the legs (hips and thighs) and sometimes the arms. This swelling is very difficult or impossible to reduce through exercise or dieting. Simply put, the fat cells grow but hardly burn the fat. The fat cells do not get smaller with weight loss.

As a result, an excessively thick layer of fat forms, especially in legs and arms. As this fat produces bad substances, blood and lymph vessels are damaged and nerve endings are stimulated. The reduced drainage releases inflammation.

Lipedema is a chronic and progressive condition that occurs in moderate to severe form in 10% of women.

How does Lipedema occur?

The origin of lipedema and the exact cause is not known. It is likely that hereditary factors and female hormones play a role. There is no indication that women with lipedema have a hormonal disorder.

What aggravates Lipedema?

By gaining weight and eating fast carbohydrates (sugar-sweetened drinks and foods), the abnormal fat cells will fill up and later fail to burn them (when losing weight). Thus, the fat cells become thicker and thicker.

Little exercise reduces muscle strength and lymphatic drainage. As a result, bad substances accumulate and will cause further damage in the tissue.

Characteristics Lipedema

  • There is (usually) an increase in the volume of the legs (and possibly arms) relative to the rest of the body starting at puberty.
  • With weight loss, the torso, breasts or face become thinner, but the legs (arms) do not.
  • Waist size is often 1 to 3 sizes smaller than that of the legs.
  • A heavy feeling in the legs (and sometimes the arms).
  • Legs (and possibly arms) are painful when touched or bumped and bruising occurs easily.
  • Cellulite appearance (orange peel).
  • Hands and feet are not thick (unless there is concomitant lymphedema), the lipedema stops above the ankle.
  • Changing walking patterns can eventually cause knee problems.
  • As exercise becomes more difficult and losing weight seems pointless, obesity can often develop in the second instance.
  • Sagging of connective tissue.

Severity and course of Lipedema

Lipedema comes in varying degrees and the course is not the same in everyone. One patient develops a minor lip edema without pain and movement restriction and remains so throughout life. In another patient, there may be a severe stage, in which severe pain and movement restriction develop. It may be a gradual process, or it may be accompanied by sudden exacerbations and periods of rest. As a result, quality of life may deteriorate.

Treating lipedema; treatment options

Treating lipedema is not limited to a single procedure. The treatment of lipedema requires a multidisciplinary approach in order to achieve the best results. This multidisciplinary approach involves the following specialists and components:

  • Nutrition plan: Guidance from a dietitian to prevent weight gain and weight fluctuations; it is unfortunately true that what weight is added to tissue affected by lipedema hardly ever comes off again. In particular, a low-carbohydrate diet (no ingestion of fast sugars) is recommended.
  • Exercise plan: Maintaining (daily) exercise and muscle strength. Individual supervision by a physical therapist through graded activity may be required.
  • Support and lymph drainage: Support lymph drainage through therapeutic elastic stockings, manual lymph drainage and pneumatic compression therapy. This can reduce volume, pain and progression of the condition and is done by both physical therapists and skin therapists. Breathing exercises can also improve lymph flow and drainage:
  • Coaching: Lipedema can be mentally draining, think of the consequences of (years of) pain, movement restrictions or living with disproportionate body proportions. In such cases, guidance from a coach or psychologist may be advisable.
  • Lipedema liposuction: Surgical treatment in the form of liposuction under local anesthesia with tumescent fluid, or tumescent liposuction.

Tumescent Liposuction for Lipedema

Would you like to have your abnormal fat evenly removed in a safe and cosmetically responsible manner without damaging your connective tissue, blood vessels, nerve branches, lymphatic vessels and skin?

Then tumescent liposuction, the right materials and an experienced practitioner are necessary.

Through tumescent liposculpture, liposuction under local anesthesia, we can permanently remove much of the (abnormal) fatty tissue, thereby reducing fat volume and almost always reducing pain and fatigue symptoms.

Lipedema liposuction

In this form of liposuction, a large amount of fluid with anesthesia is injected into the fatty tissue. This softens the adipose tissue and spares the other structures. Anesthesia or general anesthesia is not necessary with this treatment and increases the risk of complications. In addition, for optimal results, you must be able to move and even stand during the treatment. If desired, however, you may be given a tranquilizer tablet.

Removal of fat in patients with lipedema by "regular" or conventional liposuction is not recommended, according to the national lipedema guideline.

Limitations and patient safety

There is a restriction on the amount of tumescent fluid that can be injected per treatment. This gives a limit to the area that can be treated at a time. In practice, this means that upper legs should almost always be treated in 3 sessions (2 legs at a time). Lower legs can be treated in 1 session. Experience shows, however, that the limitation of the amount of fluid is rarely a problem in practice. Treating a larger area is often not desirable, as the procedure would then take too long and recovery would be more difficult.

Result of Lipedema liposuction

Depending on the amount of fat removed, the result is visible immediately after the treatment. This result is permanent: the fat that is gone stays gone. However, although much of the abnormal tissue is removed, we cannot remove everything. For example, there must always be fatty tissue between the skin and the underlying muscle. This remaining fat continues to behave as lipedema, but your lipedema will never get as bad as it did before treatment.

However, it is important not to gain weight so that you can benefit from the treatment for decades. Fortunately, exercise becomes easier after the fat and pain symptoms decrease, increasing motivation and making further weight reduction easier.

Manual fat extraction for optimal results in Lipedema

After removing the abnormal fat tissue through liposculpture, hard fat deposits may remain in the treated area. ReSculpt Clinic also looks at the cosmetic aspect. For optimal results of lipedema treatment, our medical specialists perform manual fat extraction.

Manual fat extraction involves manually removing the hard residual fatty deposits from the holes made for liposuction. In this way, the result of lipedema treatment becomes even more beautiful in a natural way.

Risks and guidelines

At ReSculpt Clinic, doctors work according to national guidelines and field standards. This includes the treatment of lip edema. You can find the national guidelines for lip edema at the Dutch Society of Dermatology and Venereology.

Tumescent liposuction does not fall under insured care and is not reimbursed by health insurance companies, despite the fact that the condition lipedema is indeed recognized as a disease.

Frequently asked questions about Lipedema

There is a time of at least 6 weeks between each treatment. This is necessary because we want one area to be healed before starting the other (adjacent) area. It is also important that you recover physically and mentally before the next treatment. We always treat the lower legs last.

If you start treating a region of the thighs at too early a stage on the lower legs (within 6-12 months), fluid from the upper legs always runs into the lower legs. This is why in lipedema we treat the upper legs first. After this, we wait at least 6 months until we treat a region of upper legs.

In upper thigh liposuction, we always treat symmetrically, meaning both thighs at the same time. For example, consider liposuction inner thighs both left and right.

There is a limit to the amount of fluid and anesthesia that can be used per treatment. Therefore, we divide the upper legs into 3 zones, we also draw this out on the legs and thus determine the regions to be treated.

The 3 regions treated at a time are:
- inner thighs and half back thighs;
- outer thighs with the other half of the back of the thighs
- front thighs.

So upper legs are treated in 3x with lipedema.

Regular sport and exercise is important in lipedema and always has a positive effect and will reduce lipedema symptoms. In this regard, be careful that when there is still plenty of lipedema present (without treatment) the chosen sport does not cause "blows" to the adipose tissue, such as running, for example.

Sports that are good for lipedema, for example; walking, swimming or aqua jogging are generally sports that are enjoyable for lip edema patients.

With lipedema, it is out of the question to use cryolipolysis, or fat freezing.
